The incompetence of the Jedi is the biggest reason for Anakin Skywalker’s fall.

Anakin was doomed to fail from the start. The Jedi ripped him away from his mother and expected him and arguably forced him to feel nothing about it. They saw Anakin’s normal feelings of love for his mother and Padme and dismissed it as evil which only made Anakin suppress his anger and resentment more.
